HOW DO I SUBMIT MY FORM?

Once filled out, please e-mail it or fax it to us.  Be sure not to leave any blank spaces.  All fields are required.  New York departments must be submitted with a tax exempt certificate.

HOW LONG DOES APPROVAL TAKE?

Once we receive your form you can expect your account to be setup within 1 business day. Our sales team will contact you with any questions or notify you once your account has been approved.

HOW DO WE PLACE OUR ORDER?

Once your account is approved you can fax or e-mail your PO. You may also call and place your order over the phone.

WHAT ARE THE PAYMENT TERMS?

Orders will be billed and are due upon receipt. Please make all checks payable to “Superior LEDs and mail to 840 Nashville St, Russellville, KY 42276.
